Annual Deductible: $1,500 Single / $3,000 Family
Coverage: After deductible, plan covers 80% (employee 20%) Out of Pocket Maximum: $3,000 Single / $6,000 Family Coverage: After max, plan covers 100% Telemedicine: Provides access to U.S. board certified physicians and pediatricians via phone or video at any time. Pharmaceutical: Pharmacy discounts are included in the medical plan and purchases apply towards the deductible. The deductible and out of pocket maximum amounts provided above will change for non-network charges. Remember to stay in-network to keep costs low. |
