Woodgrain's Health Advantage Plus Plan has an annual deductible and out-of-pocket limit amount, which include both medical and pharmacy expenses. This means that you pay the full amount that the plan has contracted with the pharmacy to charge for your prescriptions (not just the copay), until you have satisfied the deductible. Once the deductible is satisfied, your prescriptions may be subject to copayments. If you reach the out-of-pocket limit, you will not be required to pay a copayment.

Smoking Cessation Products
Woodgrain provides benefits for a smoking cessation program. Woodgrain will pay 100% of the cost, up to 180 days per calendar year (two quit attempts per person per year), for any smoking cessation product, including prescription products and over-the-counter (OTC) products. Please note that a prescription from your physician is needed for OTC smoking cessation products to be covered under the smoking cessation program. |
