Woodgrain's Health Advantage Plus Plan has an annual deductible and out-of-pocket limit amount, which include both medical and pharmacy expenses. This means that you pay the full amount that the plan has contracted with the pharmacy to charge for your prescriptions (not just the copay), until you have satisfied the deductible. Once the deductible is satisfied, your prescriptions may be subject to co-payments. If you reach the out-of-pocket limit, you will not be required to pay a co-payment.

For Woodgrain's Iowa - UHC Plan, the outpatient prescription medication benefit coverage is provided after the deductible is met and the appropriate copay applies per tier medication.
Copayment per prescription (after deductible)
Your co-payment is determined by the tier to which the United Healthcare Prescription Drug List (PDL) Management Committee has assigned the medication product. All the prescription medications on the list are assigned to Tier 1, Tier 2, or Tier 3.
The copayment does apply toward a member’s out-of-pocket maximum requirement. Only certain prescription drug products are available through mail order.
